When system logic goes wrong: Responding to compliance pitfalls in EHR implementations

Even well-executed electronic health record (EHR) transitions can introduce silent compliance risks. Our Forensic Services Practice was enlisted after a health system discovered that its newly implemented EHR system was misattributing certain federally qualified healthcare claims—assigning services to supervisory providers, regardless of who rendered care.

The culprit? A single flawed logic rule embedded deep in the system configuration. The regulatory implications were real, and the potential financial exposure was material.

Our team, led by Vice President Kristofer Swanson and Principal Patricia Pelaez, helped the client isolate the problem, reconfigure the attribution logic, and validate that the client’s corrective efforts were working as designed—bringing its compliance posture back on track, and preparing it to engage effectively with the government.
